Are you wondering how to find the specific services that will help you or a loved one remain at home?
The Boone County Council on Aging would like you to know that help is offered through the Community Care Program (CCP). The BCCA's Community Care Program helps frail older people, who might otherwise need nursing home care, to remain in their own homes, and to keep them out of a nursing home, as long as nursing home placement is not necessary.
Made available through the Illinois Department on Aging, care can be provided in your home. Services offered through the program include case management, homemaker, adult day service, and in some areas of the state, senior companion. These services are particularly helpful for older people who have mobility problems and need household and personal assistance.
Individuals who need long term care can learn more about Choices for Care at the Boone County Council on Aging at 815-544-9893 (Voice and TTY). The Boone County Council on Aging is a service provider for the Community Care Program and is overseen by the Illinois Department on Aging.
Homemaker Services
Homemakers are available to assist with dusting, assist with vacuuming, assist with cleaning the kitchen and bathroom, assist with laundry or help prepare meals for older persons who are able to perform only some basic household tasks for themselves. Homemakers also assist with personal care, such as grooming and bathing, for older persons who are moderately impaired. Assistance is also available for more complex tasks, such as meal planning and household supervision, for older persons who have difficulty managing these activities.
In-Home Care
In-Home Care provided by the Boone County Council on Aging offers home care assistants to enable older persons to remain in their own homes. The Visiting Nurses Association determines eligibility for this program and cost of care is based on income.
Eligibility
You are eligible for Community Care Program Services if:
- You are age 60 or older
- You are determined physically in need of service, meaning you are at least moderately impaired
- You are a resident of the State of Illinois
- You are a United States citizen, or a legal alien
AND
- You meet the asset requirements, which will be explained to you when the Case Manager comes to your home.
Cost
If you meet the eligibility requirements and if your family income is below the federal poverty level, the State will pay the entire cost of service; you will not be charged a fee
If your family income is above the federal poverty level, the State may still pay part of the cost of service. Your share of the cost will depend upon
- Your level of impairment
- The cost of care and
- The number of people in your family who will receive Community Care Program services.
